To prepare your Certification Application, you will need to access the appropriate Student Information Application sheet and program code that corresponds to the certification program you have completed.  Click on your degree/program and follow the instructions on the sheet to complete your application correctly.  The sheet provides the exact information needed to complete your TEACH certification application for submission to NYSED.
