Athletic Training Forms

The Sports Medicine Staff is dedicated to serving the student-athletes of all 15 intercollegiate sports teams at the State University of New York- Potsdam.  In order to best serve you, we have included the corresponding forms on this page to hopefully answer your questions and get you participating with our teams.

All athletes who plan on participating (on the roster or trying out) must have completed medical form on file in the athletic training facility prior to engaging in practice or contests.

Athletes Diagnosed with Attention Deficit Hyperactivity Disorder (ADHD)

If a student-athlete is diagnosed with Attention Deficit Hyperactivity Disorder (ADHD) and taking a prescription medication for the control of this disorder please see the criteria below.  This must be on file with the Sports Medicine Staff prior to any participation in intercollegiate athletics.

Failure to document diagnosis and prescription medication(s) for ADHD may jeopardize a student-athlete's eligibility by ingesting a substance banned by the NCAA as per bylaw 31.2.3.
