NYS Health Insurance Program Dependent Eligibility Verification Project
NYSHIP is conducting a
Dependent Eligibility
Verification Project to help ensure that every participant who receives
benefits is entitled to those benefits. You will soon be receiving a
packet in the mail at home requesting your action. You have
until October 5, 2009 to submit your Dependent Eligibility Confirmation
Form and supporting documents. Please review the packet and directions
carefully and call the NYSHIP Dependent Eligibility Project Service
Center at 1-888-358-2198 with questions. Agents are available
to answer your questions from 12:00 noon to 8:00 p.m., Monday through
Friday.
Important Notes:
• Please mark out all financial information and the first five digits
of all Social Security numbers.
• Send copies—not originals—of official documents. Original documents
will not be returned. Documentation with missing signatures, dates,
seals or other needed and applicable information will not be accepted.
• Birth certificates
must list parent(s) names. If you
have the “short form” birth certificate, which does not include the
parents’ names, you will be required to obtain the long form birth
certificate. The following web sites can help you obtain the long form
birth certificate. Please allow the necessary time to receive and
submit your documentation:

• All non-English documents must be provided with an official
translation and bear the stamp and mark of a registered translator. You
may contact a local college, university or translator services company
for assistance. You or a family member cannot translate the documents.
If you send non-English documents that have not been translated, your
dependent will be marked as incomplete until the official translation
is received.
