Pre-admission Requirements

The Pre-admission requirements apply to all students taking 6 or more credit hours. The requirements must be completed by the 2nd week in July. Spring semester records are due upon acceptance.

REQUIRED: The following pre-admissions packet must be completed.

PRE-ADMISSIONS PACKET (PDF)

The Packet Includes:

   1. Health Report

   2. Tuberculosis Screening Form

   3. Meningitis Response Form (If you are 18-years-old or older this should be submitted in BearPAWS.)
      
   4. Completed Immunization Form

Students should be aware that failure to comply with New York State health requirement laws will result in termination of Your enrollment 30 days from the first day of class for New York State residents and 45 days from the first day of class for out of state residents.

Hepatitis B Vaccine Series
Chicken Pox Vaccine Series (if you haven't had the disease)
Meningitis Vaccine
Tetanus/Diphtheria Booster (within the past ten 10 years)
Hepatitis A Vaccine Series
Human Papilloma Virus (HPV) Vaccine Series
