Group Tutor Request Form

You will contact your tutor by phone and e-mail within 3 days of receiving your tutor assignment.
You will go to tutoring sessions on time and will be prepared with materials and questions.
- You understand that your tutor is obligated to wait no longer than one-half hour.
- If you are late, your tutor may ask you to wait until the next time to come so as to prevent interruptions.
- If you cannot attend a session, you will let your tutor know in advance by phone and e-mail.
- If you do not attend 3 or more sessions or if you miss class regularly, you may be dismissed from the group.
If you voluntarily withdraw from the tutoring group, you will notify the Academic Support Office.
You give permission for the Academic Support Office and your tutor to communicate with your professor regarding your participation in tutoring and your progress in the course.

Failure to follow these guidelines may result in dismissal from group tutoring.
