Re-Activation Process

If you have been an SSS participant in the past and want to take advantage of services again this year, complete the appropriate forms and submit them in person, at the SSS Office, or on-line.

In either case, the reactivation process includes:

Completing and submitting the SSS Reactivation Form.

Completing and submitting the Student Needs Survey.

You will not be able to access services until you meet with an SSS staff member.
