Tutor Request Form

Guidelines for submitting a Tutor Request:

  • YOU MUST SUBMIT A SSS STUDENT SELF-ASSESSMENT when making your first tutor request of the semester. Your tutor request WILL NOT BE PROCESSED without the SSS Student Self-Assessment.
  • If you've never been enrolled in SSS and/or you are not currently an SSS participant, you must first fill out an Application for our program. 

  • Read "SSS Tutoring Guidelines" below. Your submission of this form indicates your willingness to work with your tutor and with SSS within these Guidelines.

  • Fill out and submit this form either online or bring a printed and completed version to 119 Sisson Hall, the Student Support Services Office between 8am-4pm weekdays.

SSS Tutoring Guidelines:

1. After your request has been submitted, see Faye Lamar, the SSS Tutoring Coordinator in 118 Sisson to activate your tutoring assignment.

2. Initiate contact with your tutor as soon as possible. Failure to contact your tutor within 10 (ten) days of your assignment may result in your tutor being reassigned to another student.

3. Contact your tutor at least 24 hours prior to when you want to meet. This gives your tutor time to prepare for your session.

4. Attend classes and scheduled tutoring sessions. Your sessions may be suspended or terminated if you are not attending class or tutoring sessions. Attendance is checked on a regular basis.
