Student Teaching Information Form 

Advice for completing the Student Teaching Information Form

Here are four methods for completing the form successfully that are proven to work.

Windows 7

Right-click the link and choose to save the linked file. Open the file in Adobe Reader, fill out the form, and choose "Save" from the file menu.

Macintosh

Option 1: Using the Safari browser, click on the link to open the file, fill out the form, and choose "Print" from the File menu. Using the PDF button in the lower left corner of the print dialog, choose "Save as PDF…"

Option 2: Right-click (or Command-click) and choose to save the linked file on your computer. Double-click the file to open (or right-click or command-click the file to choose an application).

• If the file opens in Preview, fill out the form, and choose "Print" from the File menu. Using the PDF button in the lower left corner of the print dialog, choose "Save as PDF…"

• If the file opens in Adobe Reader, fill out the form and choose "Save" from the file menu OR choose "Print" from the File menu. Using the PDF button in the lower left corner of the print dialog, choose "Save as PDF…"
