Dear students,
To ensure that we have up-to-date COVID-19 immunization records for all returning students, SUNY has launched a SUNY Student Vaccination Pass App.
The SUNY Student Vaccination Pass validates your information against what is in the New York State Immunization Information Systems and the Citywide Immunization Registry for a match, and also provides a secure way for students to manually upload your records.
All SUNY students are required to log in to the system before returning to campus, as follows:
- Go to https://suny.edu/StudentVaccinationPass
- Choose “Potsdam” from the dropdown menu.
- Log in using your SUNY Potsdam campus computer account username and password
- Submit your first name, last name, date of birth, ZIP code and phone number.
- If you attend more than one SUNY campus, you may also select your other campus from the dropdown list as well.
- The system will attempt to automatically verify your vaccination record.
- If your records are located, you will see a screen that says you are verified. There is nothing further that you need to do!
- If your records cannot be located (usually because you are unvaccinated, if you were vaccinated out of state, or because it has been less than 15 days since your last dose), you will have to manually upload proof of vaccination. Once uploaded, your records will be checked first by Student Health Services before you can be verified in the system.

What if I already sent my records to Student Health Services?
You still need to log in to the system.
Do I still have to log into this system if I already have the Excelsior Pass or the NYC Covid Safe app?
Yes.
What if I can’t find my vaccination card?
If you have lost your vaccination card, you must contact your vaccination provider or your state health department to request a new copy.
What if I am partially vaccinated or unvaccinated?
You still must log into the system and complete the form. All unvaccinated or partially vaccinated students must complete the Temporary Accommodation Application before returning to Potsdam. They may also submit a Medical Exemption Form or a Religious Exemption Form if desired. Requirements and expectations for unvaccinated and partially vaccinated students can be found on the Potsdam Prepared site.
