ALL Study Abroad forms and supporting documentation should be uploaded using our secure file drop! Before submitting please be sure to retain a copy for your records.
To apply to a study abroad program sponsored by SUNY Potsdam, students must complete and submit the following forms (available in Adobe PDF format):
- SUNY OAP Application (PDF)
- SUNY OAP Study Statement (PDF)
- SUNY OAP Reference Form 1 (PDF)
- SUNY OAP Reference Form 2 (PDF)
- SUNY Conduct Review Form (PDF)
- Official transcript for all Non-SUNY Potsdam students

Students who have been accepted into a SUNY Potsdam study abroad program must complete and submit the following forms:

- SUNY Potsdam Financial Aid Arrangements Form (PDF)
- Agreement and Acceptance of Risk Study Abroad (PDF)
