  • Award Checklist - List of requirements related to specific financial aid awards
  • 23-24 application for Aid for Part-time Study (APTS) (PDF) - Please review your eligibility here. Fall/Full-year deadline to apply is September 22, 2023; the Spring deadline to apply is February 16, 2024. The application can be submitted to our secure file drop.
  • Billing/Refund Authorization - Authorize the Bursar's Office to pay the charges on your student bill with your Financial Aid.
  • FAFSA Signature Page – Complete this form if you or your parent did not sign your FAFSA with an FSA ID. The Original must be submitted, faxed and scanned copies will not be accepted.
  • SAP Appeal (Unsatisfactory/Failed Probation) - Is your SAP Status "Unsatisfactory" or "Failed Probation" with new circumstances? Review this form to see if you are eligible to appeal your SAP status.
  • SAP Appeal (Over Credit or Mathematically Impossible) - Is your SAP Status "Over Credit" or "Mathematically Impossible"? Review this form to see if you are eligible to appeal your SAP status.
  • Student Consortium - Request for financial aid when enrolling in courses at another college. Courses must be applicable to your Potsdam degree to qualify for federal aid consideration.

  • Affidavit - Certification of true, exact, and complete copy of original documents
  • Citizenship Status Clarification - Request for documentation to clarify citizenship and eligibility for federal financial aid.

  • Special Circumstances - Request for consideration of special circumstances, such as reduction of income, loss of job or change of marital status.
  • Budget Adjustment - Provide documentation of actual costs to determine if you are eligible for additional aid.
  • Status Change - Report enrollment changes to the Financial Aid Office with the Status Change Form
  • No Parent Data provided on FAFSA - Clarification of student's response to not providing parent information on the FAFSA.
  • Parent Non-Tax Filing Form - Complete this form if you are not able to get official confirmation of non-filing from the IRS and you have exhausted all other options.
