Forms & Documentation

ALL Office of the Registrar forms and supporting documentation should be uploaded using our secure file drop! Please do not email any sensitive personal information such as your social security number, driver's license, birth certificate, etc. Should you be asked to provide such information, upload sensitive documentation using the Office of the Registrar’s file drop where it will remain secure, encrypted and confidential.

Note: Uploading sensitive documents does not automatically alert Registrar staff. Please email registrar@potsdam.edu after uploading a document to alert Registrar staff.

Before submitting please be sure to:

1. Include Student’s Name and Potsdam Identification Number (P#) in the file name.
   • * Documents that do not reference the student’s name and P# in the file name will be processed after others.
2. Retain a copy of the document for your records

Forms for Student Use

Academic Forgiveness Petition 
The intent of the Academic Forgiveness policy is to allow readmitted students who previously accrued a record with a substantial number of grades below 2.0 to be “forgiven” for their earlier performance, if they meet the stated criteria. For additional information and a link to the online form, visit the Academic Forgiveness Petition webpage.

Add/Drop Form
Visit the Add/Drop Procedures webpage for information about Add/Drop (including overloads and overrides) and a link to the online Add/Drop form.

Address / Legal Name/ Legal Gender /Social Security Number Change Form
Complete the appropriate section/sections of the Address / Legal Name/ Legal Gender /Social Security Number Change Form (PDF) to change your address, name or social security number.

Appeal Form
Complete the Appeal form (PDF) to appeal a course grade.

Application for Bachelor's or Master's Degree
Students must file an Application for Degree during the semester prior to their final semester or ASAP in order to graduate. You can find the application in BearPAWS>Student Services & Financial Aid>Student Records>Apply to Graduate.

Citizenship Change Form 
Complete the Citizenship Change Form (PDF) to change your citizenship status.

Cross-Registration
For information about Cross-Registration, including the Associated Colleges regulations and much more, visit the Cross-Registration webpage.

Incompletes
Find out how to request a grade of incomplete and access the online form on the Incompletes webpage.

Late Check-In Form
Complete the Late Check-In form if you have missed the electronic Check-In deadline and can no longer complete the Check-In using BearPAWS.

Major/Minor Declaration Request
For information on how to add or change your major/minor, visit the Major/Minor Declaration webpage.

Personal Information
For information on how to add/modify/remove personal information (including gender identification and/or personal pronoun), visit the Gender Markers and Preferred Name webpage.

Post-Degree Readmission Application
Visit the Readmission webpage to apply for readmission if you have already received a degree from the College.

Readmission Applications
Visit the Readmission webpage and complete the appropriate form to apply for readmission to the College.

Residency Status Form
Complete the Residency Status form to apply for residency status for billing purposes. Visit the Residency Requirements webpage for more information and access the form.

Senior Request Graduate Course Form
Complete the Senior Request Graduate Course Form to request to take graduate courses for graduate credit during the final semester of undergraduate study (for SUNY Potsdam students only).

S/P/U
Complete the form to request a change in grade mode to S/P/U. Visit the S/P/U webpage for more information and access to the form.

Withdraw from a Course

Withhold Directory Information Form
